USE OF THIRD-PARTY SERVICES

You may choose to use other services provided by third parties in conjunction with your use of MyChart. Such services may include, but are not limited to, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor KDMC have control over the contents of these services, and neither Epic nor KDMC accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information being used and/or stored by a third party. If you decide to access any such services in conjunction with your use of MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.

MyChart gives you easy online access to portions of your personal health record and related information. You can log on to communicate with your healthcare team, cancel an appointment, request prescription renewals or manage online medical records.

Medical Information in MyChart. Information on MyChart is meant for use only to support your relationship with your physicians and other care providers, and is not your complete medical record. You may contact KDMC directly for official copies of your medical record.

The medical information in MyChart is provided "as is" without warranty of any kind. But if you find mistakes in the medical information in MyChart, please contact Scott Hill, Privacy Officer and Vice President Safety, Security and Transportation for KDMC, at 2201 Lexington Avenue, Ashland, Kentucky 41101, phone number 606-408-4488, and e-mail scott.hill@kdmc.net. You will be provided with the necessary form to report any information that you believe is not correct in MyChart.

The medical information provided in MyChart is not a replacement for proper medical care. Contact your clinic, your physician or 911 services immediately if you need medical attention.

Privacy and Security of MyChart You will create a unique identification (ID) code and password to be used to access your health information via KDMC's MyChart. This ID and password are unique codes that identify you in the MyChart computer system. It is extremely important that you keep your MyChart ID and password completely confidential. If at any time you feel that the confidentiality of the password has been compromised, you should change it by going to the Password link on the KDMC MyChart website. KDMC takes no responsibility for and disclaims any and all liability or consequential damages arising from a breach of medical record confidentiality resulting from you sharing or losing your password. If KDMC discovers that you have inappropriately shared your password with another person, or that you have misused or abused your MyChart access privileges in any way, your participation in KDMC's MyChart may be discontinued by KDMC without prior notice.

KDMC cannot be held responsible for:

a)Security of electronic communications if you do not follow our requirements for electronic communications;

b)Unauthorized disclosures of your information if you do not log out of an active session;

c)Unauthorized disclosures of your information if you give your user ID and password to someone, or if they are lost or stolen;

d)Unauthorized disclosures of your information if you print information from MyChart;

e)Unauthorized disclosures of your information resulting from problems with your personal computer; or

f)Other events that are outside the control of KDMC.

Appropriate Use. You are granted access to MyChart for the purpose of furthering your medical care. You are not permitted to use inappropriate or abusive language in MyChart, or to use MyChart in a way that causes a disruption to KDMC or your physician's office operations. If you use MyChart for a purpose unrelated your medical care, KDMC may suspend or terminate your access privileges.

Other Terms. KDMC is not be liable for any damages suffered by you or other third parties that are directly or indirectly related to the information contained in MyChart, including direct, indirect, special, incidental or consequential damages of any type, even if informed of the possibility of such damages. KDMC cannot guarantee access to the MyChart system and is not liable for your inability to access or view the information contained on the site.

You must be age 18 or older to request a MyChart account. Parents or guardians of minor children may request access to the minor child's account, with some limitations. If you want access to your child's account, you will need to fill out a Pediatric Proxy Access Consent Form and have it signed by your child's physician. Legal guardians or medical attorneys in fact for an adult, with the ability to make medical decisions for that adult, may request access to his or her account. If you want access to an account for an adult under guardianship, or for whom you have a medical power of attorney, you will need to fill out an Adult Proxy Access Consent Form. Pediatric and Adult Proxy Access Consent Forms may be obtained on the MyChart website.

This Agreement is governed by the laws of the Commonwealth of Kentucky and any dispute arising under this Agreement must be submitted to binding arbitration in Ashland, Kentucky, pursuant to the Commercial Arbitration Rules of the American Arbitration Association.

MyChart information and the logos, icons and other marks shown or referenced within are reserved by KDMC and are protected by copyright and trademark law. You may not reproduce such information without the prior written consent of KDMC.

The terms of this Agreement may be changed at any time in the discretion of KDMC, by posting new terms and conditions for the use of MyChart on the KDMC MyChart website.

KDMC may terminate the MyChart program, or terminate or suspend your access privileges, at any time. If you wish to terminate or suspend your access privileges, you may do so at any time by sending your request to the following email account. mychart@kdmc.net

Why can't I log into MyChart? ›
